 Offer Details

Receive a complimentary dental implant consultation with Dr Sachi Fernando at Moonee Ponds Dental Group (Initial Consultation), plus $1000 off implant treatment if you proceed with treatment following your consultation. A consultation will be required before any later treatment is performed. You will be required to fill out a medical history form when you attend the Moonee Ponds Dental Group practice (and prior to your consultation) to assist Dr Sachi Fernando’s evaluation. There is no obligation for you to proceed with the dental treatment following the consultation. All treatments are subject to the Dr Sachi Fernando's clinical assessment of suitability for you.

During the Initial Consultation, Dr Sachi Fernando will:

1. discuss your goals;
2. provide information about implant treatment options;
3. evaluate your teeth condition to consider whether implant treatment may be suitable for you; and
4. if the treatment is deemed to be suitable for you, discuss payment options and provide you with a quote containing an estimated cost for the treatment.

The $1,000 discount will only be applied to your treatment cost if:

• you are deemed clinically suitable for implant treatment by Dr Sachi Fernando following assessment during the Initial Consultation; and
• you proceed to book and complete implant treatment at Moonee Ponds Dental Group.

The consultation does not include examination, x-rays or other dental treatments that may be required before commencing implant treatment. Moonee Ponds Dental Group does not have CBCT scanning equipment at the practice. If CBCT scans are required for your implant assessment or treatment planning, you will be referred to an external X-ray centre where you will be charged directly at that provider's fees. These external scanning costs are not included in this offer or the implant treatment quote. The $1,000 discount applies to the total cost of implant treatment only and cannot be redeemed as cash or applied to other dental services.

Offer Period

This Offer is valid for patients who make an appointment at Moonee Ponds Dental Group during the period between 7 November 2025 to 30 April 2026, subject to appointment availability. 

Redemption Process

To claim this Offer:

• Attend your scheduled appointment
• Complete the Initial Consultation where suitability for treatment will be assessed

Offer Restrictions

Dental implants are not available for purchase by the general public. They will only be offered following the consultation process as set out in this Offer.

The following restrictions apply to the eligibility and use of the Offer:

• you must be a new or existing patient of Moonee Ponds Dental Group;
• only one Offer can be redeemed per person;
• the $1,000 discount is only available if you are deemed suitable for treatment by our dental professionals following clinical assessment during the Initial Consultation;
